For all programs we operate at Leaside Arena, safe protocols are in place so we can operate our programs safely for you.

COVID-19 WAIVER

The Covid-19 Waiver, Release, Assumption of Risk, and Indemnity regarding Covid-19 is on our registration form that all participants must agree with to be eligible for any Hockey Extreme program. All youth programs are consented by an adult who registers their child(ren) and an adult who registers themselves for an adult program. Clink link to review.

REFUND & CANCELLATION POLICY -  UPDATED MARCH 1, 2022

1.  Full refund will be provided with the following guidelines.

a) If Hockey Extreme is not allowed to operate their business because we are closed down by the province or Arena, you will be refunded 100% from that date onward until we are allowed to begin operation.

b) Youth Summer Day Camps you must cancel before June 1st for full refund minus a $75 administration fee

c) Youth PA Day Camps you must cancel 1 month before the start of the camp for full refund minus a $25 administration fee

d) Youth Spring Weekly Programs you must cancel 1 month before the start of the program for full refund minus a $25 administration fee

d) Youth Full Week Camps (excluding summer, see 1b.) you must cancel 1 month before the start of the camp for full refund minus a $75 administration fee

e) Adult programs you must cancel 1 month before the start of the program for full refund minus a $75 administration fee

2. Cancellations made within 1 MONTH to the day before the start of a camp/league/program with a doctor's note only.

a) Youth PA Day Camps: $25 administration fee + credit towards future program, 6 months to use

b) Youth Spring Weekly Programs: $25 administration fee + credit towards future program, 6 months to use

c) Youth Full Week Camps: $75 administration fee + credit towards future program, 6 months to use

d) Adult programs: $75 administration fee + credit towards future program, 6 months to use

3. Cancellations made with a camp/league/program already in progress.

NO REFUNDS OR CREDITS for sessions in progress, missed due to illness, holidays, competitions, injuries or management disputes. You are responsible for knowing the programs you registered for and if you show up or not, your hours at the camp/league/program are considered used. Hours are non-transferrable from one participant to another. However, we will work hard to try and sell your spot. If successful, you will receive a CREDIT (6 months to use) equal to the same value as the new participant paid minus a $25 administration fee.

IMPORTANT TO KNOW AS WELL

a) Full Hockey Equipment is Mandatory for all programs.

b) Hockey Extreme reserves the right to decline the applicant enrolment in the camp/league/program and terminate participation in the camp/league/program without refund for conduct by the participant detrimental to the best interest of the camp/league/program and other participants.

c) PHOTOS & VIDEOS. Photos and videos at a camp/league/program may be taken by Hockey Extreme and may be used for promotional purposes.

d) NSF CHEQUE: An administration fee of $50 will apply to all NSF cheque(s). Who pays by cheque anymore?
